Job Description:
Join our dedicated team as a Nurse Coach, where your clinical expertise will be instrumental in managing Post-Acute Care patients’ transitions from acute care settings to home care. This telehealth role requires you to engage with patients through telephonic outreach, providing education, coaching, and care coordination to achieve the goals of our Post-Acute Care Program, which services four different health plans.
Your day-to-day responsibilities will include engaging patients, administering assessments, coordinating with home health agency nurses and physicians, and documenting all interactions. Our program aims to optimize patient care by managing up to 90-day episodes, commencing prior to discharge or hospitalization for elective surgical cases, and facilitating safer, faster home transitions.
This is a full-time, work-from-home position that requires evening and weekend coverage, subject to business requirements. A comprehensive training program will be provided to ensure your success in the role.
Hard Skills:
- Compact RN license required
- Minimum of 5 years of nursing experience, with at least 1-2 years in an acute setting
- Experience in telephonic case management/triage
- Proficiency in navigating multiple computer systems/screens
- Registered Nurse with a current and unrestricted license for the state of practice
- Expertise in telephonic Case Management
- Demonstrated ability to learn new Electronic Medical Record systems
Soft Skills:
- Strong communication skills for day-long phone interactions with members/patients

Tues – Sat; 8:00am – 5:00pm
Additional Information:
Employee Value Proposition (EVP): Enjoy 6 paid holidays and 5 days of PTO after 30 days on-contract. The position is 100% work-from-home, with all necessary equipment provided. Initial training is 4-6 weeks, Monday to Friday, 8am-5pm EST. There is a no time-off policy for the first 60-90 days. Calls to members are made individually, not through an auto-dialer, and comfort with sitting/working at a computer for 8 hours a day is essential.

This is a contract-to-hire position, with an initial contract duration of 12 months.
